Address the cover sheet to the exact department (for example "Attn: Estate Services" or "Attn: SCRA Specialist"), include your name as it appears on the account, your account or loan number, a callback phone number, the total page count, and any required authorization or legal documents. Avoid putting your full Social Security number on the cover sheet unless the bank's own form requires it.
